• Intake

    Clients are asked to complete about 15 minutes of paperwork before their first appointment. There are two options:

    • Plan to arrive 15 minutes early to complete paperwork in the office, or
    • Print out the forms in the "Intake Forms" section below, complete them at home, and bring them with you to the first appointment.

     

  • Insurance

    If you are planning on using your health insurance, you will need to complete the additional forms found in the "Additional Forms for Insurance" section below. You will either:

    • Pay for your first session out of pocket, bring your completed "Insurance Benefits Request Info" form to your first appointment, and wait to see if your insurance covers services, or
    • Email your completed "Insurance Benefits Request Info" to me at least 3 business days prior to your first appointment so our billing specialist can get an estimate of your benefits

  • Confidentiality

    Confidentiality is of utmost importance in counseling. What you say in your sessions is completely confidential, with the following exceptions:

    • Client reports a plan to harm self or other
    • Client reports child abuse or elder abuse 

    It is required by law that these must be reported to authorities. 

  • Cancellation Policy

    Once you set up an appointment, that hour is reserved just for you. We ask that you notify the office 24 hours in advance if you plan to cancel your appointment, so we can schedule another client in that time slot. Clients are charged for missed appointments and appointments cancelled with less than 24 hours notice, except in the case of emergencies.
